COMMUNITY-BASED CASE MANAGER

What we need you to do:

A Community-Based Case Manager promotes positive growth for clients through case management services by visiting with clients, performing assessments, facilitating supervised visitation between parents and children, providing support, and assisting with clients’ needs as they coordinate services with other providers involved with the clients’ care. Our services are provided to adolescents, children, and families in the client’s communities and homes. Clients are referred to the agency by the Indiana Department of Child Services or by Juvenile Probation Departments.

Compensation:

Range of $39,000 to $44,000 based on education and experience, with potential for overtime.

These are the qualifications we are looking for:

  • Bachelor’s degree in Social Work, Psychology, or Human Services from an accredited university (other Human Services degrees will be considered, depending on the coursework completed);
  • Valid driver’s license and the ability to be covered by the agency’s auto insurer;
  • Must be at least 21 years of age;
  • Ability to pass multiple criminal background checks;
  • Knowledge in out-of-home placements, child abuse/neglect, child & adult development, strongly preferred;
  • Strong assessment, communication, and documentation skills required.

Weekly Schedule:

Full and part-time opportunities are available. Contracted work would also be considered. A full-time schedule would include Monday through Friday, predominately first shift hours—with flexibility and adaptability to some degree to the clinician’s preferences. Some evening hours to accommodate clients’ scheduling needs and acute circumstances are expected.
